In some cases, orthognathic surgery may be necessary to achieve an ideal esthetic and functional orthodontic outcome.

In cases of severe overbite or underbite, an oral surgeon can adjust the position of the maxilla and/or mandible to create a better relationship.

A surgical approach may also be indicated if there is an asymmetry; for example, uneven ramus heights, or a maxillary cant.

A patient with vertical maxillary excess and/or an open bite can achieve correction through oral surgery that impacts the maxilla.

Last, a surgical approach may be recommended for improvement of chin projection from the profile view.
